Te whiwhinga mahi | The Opportunity

Do you have excellent clinical assessment skills, wish to expand your knowledge and be part of a passionate, high functioning nursing team within the community, where no two days are the same?

The Intermediate Care Team is part of the Community and Long-term Conditions Directorate and compromises of the Rapid Community Access Team (RCAT), an intensive, short term service that provides acute Nursing and Allied Health care for adult patients and the Interim Care Scheme (ICP) providing access to beds in Age Related Residential Care facilities (ARRC) to support patients who don’t need to be in hospital, but are not yet ready to be supported in their own home.

Being part of the Intermediate Care involves:

  • Working within a multidisciplinary team of health professionals and have the privilege of working with patients within their own environment.
  • Nursing and Allied health input that supports the transition from hospital to home.
  • Supporting our Interim Care patients.
  • Supporting the acute care needs of people in the community to help prevent further crisis and/or deterioration in their condition.
  • Use of rehabilitation skills to maintain functioning during a period of care.
  • Holistic patient centred care.
  • Support the improvement of the integration of community care models.
