We are seeking an enthusiastic, motivated and NZ Registered Health Professional to join out Te Roopu Āramuka Whāroaroa - CCDHB-s Homeless Team based in Newtown.

The Plan Coordinator works collaboratively using a Strengths Based and Trauma Informed Approach to identify the person-s goals, strengths and challenges. They advocate for service engagement and the coordination of resources to meet the person-s goals and health and social needs. 

As a Plan Coordinator you must be able to show a good understanding of the cultural, social and economic pathways that lead to people-s experience of homelessness and lead a compassionate response. Your role will include addressing barriers to access coupled with staunch advocacy.

Ä€ramuka is a small and committed team that works with people who experience homelessness who have high and complex unmet health and social need. Ä€ramuka helps to integrate a diverse range of services, coordinates support and facilitates change for individuals where this has previously not been possible. We are embedded in the Intensive Recovery Sector of 3DHB MHAIDS. 

Moū - About you

The ideal applicant must have a relevant professional qualification, registration with a professional body and a current practising certificate. Excellent communication skills and the ability to engage and negotiate with professionals, colleagues and peers in the health and social services environment is essential.
