Fixed Term Position Full-Time and Part-Time Roles
We are looking for experienced Nurse Practitioners to provide clinical expertise and leadership across the Community Health Service. In this role you will work to support acute care in the community through our Hospital in the Home model of care. We have established a COVID Hospital in the Home pathway to provide community-based monitoring and support for at risk individuals while they recover. You will provide support and intervention across both COVID and standard acute care pathways within the Counties Manukau community.
You will be required to initiate and develop innovative approaches to problem solving while utilising skills from other disciplines and team members to co-ordinate patient care. The NP will work with Locality Community Health Teams within a care coordination framework in partnership with primary care and with patients/family/whaanau, to provide high quality patient-centred care.
You will be empowered to make decisions or recommendations relating to patient management, while demonstrating a high level of communication and professionalism.
