1.0FTE (80 hours per fortnight ) Permanent full time position

Suitable for Nursing & Allied professionals.

The Complex Discharge Coordinators (CDC) primary role is to facilitate and coordinate safe, timely and patient centred discharge.

Are you a health care professional - nursing or allied - who is able to receive referrals for patients who are deemed complex and work with the MDT from preadmission, acute admission through to discharge?

The CDC will focus on: timely, efficient use of resources, effective discharge planning, and be a leader in effective interdisciplinary communication and patient & whanau involvement in goal setting and discharge planning.

Emphasis placed upon: board rounds, leading the weekly MDT meetings, discharge before 11am, decreasing length of stay and mitigating readmissions.  The CDC will need to lead the facilitation of management plans.

The casual CDC will be required to work:

• Monday - Friday 0800 - 1630 (some flexibility to meet patient and whanau needs may be required)
  
• to facilitate annual leave and study leave for our permanent medical CDC
