This position covers from Plympton up to Dry Creek and across to Hindmarsh and all the regions in-between. 

It is a part-time position which will be working 3 days a week.

You will be responsible for: 

• In this position you will conduct face to face home support assessment with clients daily using a standardized national assessment process, with a focus on wellness and reablement. 
• Working closely together with clients to de develop individualised support plans to achieve wellness and healthy ageing goals. 
• Match and refer clients to appropriate support services. 
• Ideally service clients in surrounding areas  
• Contribute to process of continual improvement by always seeking better ways to assist colleagues and clients.
