This is a Full Time position covering the Orange, NSW region. 

In this role you’ll

• Conduct face to face home support assessments with clients on a daily basis using a standardized national assessment process
• Work together with clients to develop individualised support plans to achieve wellness and healthy ageing goals
• Match and refer clients to appropriate support services
• Ideally service clients in surrounding areas
