The opportunity:

APM is excited to be part of the Federal Government Assessment and Referral Initiative for older Australians to access the care they need to remain living independently in their chosen community.  We are committed to excellence in service delivery and looking for people with a passion for aged care and enabling better lives.  

You will be responsible for: 

  • In this position you will conduct face to face home support assessment with clients daily using a standardized national assessment process, with a focus on wellness and reablement. 
  • Working closely together with clients to develop individualised support plans to achieve wellness and healthy ageing goals. 
  • Match and refer clients to appropriate support services. 
  • Ideally service clients in surrounding areas  
  • Contribute to process of continual improvement by always seeking better ways to assist colleagues and clients.
