You will work in the community, in an assessment and rehabilitation capacity to help older people and people with complex disabilities to remain independent in their own homes for longer and also supporting those transitioning home from hospital. You will be part of a multi-disciplinary team working in partnership with General Practice and other community service providers to come up with creative solutions for complex situations. You will be part of a progressive, award winning model of care.
EMS accreditation for Occupational Therapy in personal care and household management is essential. Accreditation in basic/complex housing modifications and level one wheeled mobility and postural management is preferred but not essential; an interest in working towards these areas of accreditation is desirable.
