Reporting & Compliance
• Maintenance of accurate record keeping on client database (CMS)
• Meet key performance indicators within required timeframes
• Provide information about support needs and alternate referral pathways as required
• Identify child protection issues and report concerns to Child Safety
• Check client pack completeness, accuracy, and availability
• Data registry and uploading of records to Care Master system
• Establish client folder and maintain updated client tracker folders
• Conducts weekly house visits monitoring and evaluation to ensure compliance in the implementation of ANZ Care service delivery unit process system. Provision of care and support to our clients
• Coordinate with Team leaders and Ensure all workers are properly informed about clients’ needs and daily routines via induction process.
• Implementing and monitoring NDIS plans, assisting participants identify and achieve goals through strategy development and implementation; and
• Will work with us on exciting new accommodation projects
• Meet key performance indicators within required timeframes
• Ensure positive outcomes for new participants to ANZ Care or
