Job Description

In these roles, available on full time or part time basis, you'll be part of a positive, cohesive group of experienced practitioners, applying your combined expertise to conduct assessments that keep your clients' needs central to the work that you do.


Uniquely, direct care won't be part of your day-to-day - instead, you'll apply your experience to an approved process that sees you:

  • Conduct desktop reviews of existing assessment documentation.
  • Conduct interviews with residents, carers and other relevant people
  • Collaborate with other practitioners in your team to accurately complete AN-ACC funding assessment tools
  • Combine your findings to assess the appropriate funding classification for restorative care / reablement.

This is a part time position working 4 days a week.


Locations
These opportunities will see you visiting Aged Care facilities across the Yorke, Lower North and Barossa Region. You will be working from Gawler to Goyder across to Wallaroo and out to the Yorke Peninsula. Please note that there will some travel reimbursements for this position.

To be considered, you will need:

  • 5 years’ experience in aged care or a related area of health care
  • Relevant Tertiary qualifications in Nursing, Physiotherapy or Occupational Therapy
  • Current unrestricted General Registration AHPRA
  • Demonstrated Australian aged care experience including working with people with dementia
  • Demonstrated experience conducting clinical assessments using a range of assessment tools
