Job Description

Looking to be at the forefront of change in the Aged Care industry and are an aged care professional with 5 years’ experience? these roles, once you have completed your 3-day training, will see you be part of a positive, cohesive group of experienced practitioners, applying your combined expertise to conduct assessments that keep your clients' needs central to the work that you do. 

Working as part of the larger regional team you will travel within your region, with a member of your assessment team, to assess aged care facility residents. 

This is a full-time permanent position.

Conduct resident assessments that include:

  • Reviewing documentation 
  • Collaborating with practitioners in your team to accurately complete AN-ACC funding assessments
  • Conduct interviews with residents, facility staff and family members 

Locations 
These opportunities will see you visiting Aged Care facilities across the New England Region. You will be working from Armidale across to Tamworth. Please note that there will some travel reimbursements for this position. 

To be successful in this position we are looking some someone who is located within this region.

To be considered, you will need: 

  • 5 years’ experience in aged care or a related area of health care 
  • To be either a Registered Nurse, Physiotherapist or Occupational Therapist
  • Current AHPRA Registration
  • Demonstrated Australian aged care experience including working with people with dementia
  • Demonstrated experience conducting clinical assessments using a range of assessment tools 

...and soft skills related to motivation, organisation and working independently and as part of a team.
